Diabetes and weight loss drugs like Ozempic and Mounjaro carry a small, mostly reversible hair loss risk, about 7 cases per 1,000 yearly, a large Penn Medicine study in The BMJ finds.
Adults with type 2 diabetes who started on Ozempic or Mounjaro were more likely to be diagnosed with hair loss than patients on two older classes of diabetes drugs, a Penn Medicine study published in The BMJ on July 22 finds. The relative risk was statistically significant, but the absolute numbers stay small and the shedding is mostly reversible.
Researchers led by Yong Chen at the University of Pennsylvania compared 12,004 patients who started a GLP-1 receptor agonist (the class behind Ozempic, Mounjaro, Wegovy, and Zepbound) with 15,221 who started an SGLT-2 inhibitor, and 11,964 who started a DPP-4 inhibitor. Both comparisons pointed the same way.
Hair-loss diagnosis rates ran about 6.9 per 1,000 person-years on GLP-1s versus 5.0 on SGLT-2 inhibitors, and 6.5 versus 3.9 against DPP-4 inhibitors, roughly one extra case per 200 patient-years. The hazard ratios were 1.37 (95% CI 1.08–1.73) and 1.68 (95% CI 1.28–2.20), MedPage Today reports.
The predominant phenotype was non-scarring alopecia, hair loss that can grow back. Researchers attribute the signal to telogen effluvium, a temporary shedding most plausibly driven by rapid weight loss and caloric deficit rather than direct follicular damage, StudyFinds notes.
Two caveats: the design is observational, not a trial, and the cohort covers only type 2 diabetes patients. Whether the effect shows up in the much larger population using these drugs purely for weight loss remains an open question.
