The next generation injectable drove strong weight loss at 80 weeks, but a secondary cardiovascular endpoint didn't reach significance in a trial Lilly says wasn't powered to answer it.
Eli Lilly's retatrutide a next-generation injectable obesity drug, cut weight by 22.6% at 80 weeks in roughly 2,000 people with severe obesity and established cardiovascular disease. It is not a heart drug yet. The difference between those two sentences is the next 18 months of obesity-drug news.
The Phase 3 trial enrolled participants with obesity and cardiovascular disease, with or without diabetes. On the highest dose, they lost 22.6% of body weight compared with 3.2% on placebo over 80 weeks. That figure is the largest weight loss reported in a late-stage obesity-drug trial to date.
The trial's secondary cardiovascular endpoint did not reach statistical significance against placebo. That endpoint tracked MACE, the composite of cardiovascular death, heart attack, and stroke. Lilly framed the result by noting the trial was not powered for cardiovascular outcomes and that fewer MACE events occurred than the company had anticipated. A low event rate cuts statistical power, and "fewer than expected" can describe either a real effect or an underpowered test.
Lilly separately reported two additional Phase 3 successes within its TRIUMPH program, hitting its weight and A1C targets. The class story now has a consistent efficacy record and a single open question: does the weight loss translate into fewer heart attacks, strokes, and cardiovascular deaths?
That question will be answered by trials built to answer it. SELECT, the dedicated cardiovascular outcomes trial for Novo Nordisk's Wegovy, and SURPASS-CVOT, the analogous test for Eli Lilly's tirzepatide, are the established comparators. Retatrutide's dedicated CVOT is the next readout that will move the class in either direction.
Today's result is strong efficacy, with the cardiovascular-outcomes question deferred to a properly powered trial. The next read that resolves it is the dedicated retatrutide CVOT.
