The FDA cleared Johnson & Johnson's Ottava robotic surgical system on July 22, 2026, for a defined list of soft-tissue operations, and J&J is launching it only with select U.S. hospitals first. Ottava is the first surgical robot with four robotic arms integrated into a standard operating table, a design that drops the separate boom and carts that have defined the category since Intuitive Surgical's da Vinci went on sale in 2000 (J&J press release).
Robbie Marcus, the J.P. Morgan analyst who covers the sector, told trade publication FierceBiotech that "while Ottava has some interesting selling points (smaller footprint), our doc checks continue to point to Intuitive as the clear favorite given its physician relationships and entrenched integration into the hospital workflow and academic programs" (FierceBiotech). That is the on-record counterweight to J&J's "next era in surgery" language, and it points to where this competition will actually be decided: not in engineering, but in the slow grind of surgeon training, hospital procurement, and academic-program lock-in.
The cleared indications are a defined set of upper abdominal general surgery procedures: Roux-en-Y gastric bypass, gastrectomy, cholecystectomy, splenectomy, gastric sleeve, small bowel resection, appendectomy, lysis of adhesions, fundoplication, and hiatal hernia repair (J&J press release). The system is not cleared for urology, gynecology, or thoracic work, which is where Intuitive's da Vinci still has the deepest installed base. J&J is running a U.S. clinical trial in inguinal hernia repair, a high-volume procedure that the company will want on the label before it can take a real run at Intuitive's volume.
J&J filed for De Novo classification on January 7, 2026 (J&J submission announcement), the FDA's pathway for novel devices without an existing predicate, and the FDA's authorization creates a new device classification for table-integrated soft-tissue surgical robots. J&J gets first-mover classification, while a faster 510(k) route for future competitors with similar architectures is the cost: any future table-integrated soft-tissue robot can now cite Ottava as a predicate in a 510(k) submission, which is faster and cheaper than another De Novo.
The market Ottava is walking into is dominated by a company that has spent two decades on training pipelines, not just sales. MDDI Online pegs Intuitive's share of the global soft-tissue market closer to 80% (MDDI Online). J&J's competitive set includes Medtronic's Hugo and CMR Surgical's Versius, both of which have been working the same smaller-footprint angle with varying commercial success.
J&J executives called the clearance the start of a "new category" of soft-tissue robotics and the opening of "the next era in surgery" (J&J press release). The trade-press coverage mostly echoed the marketing line. The economic signal is smaller than the language. A limited rollout to select U.S. customers, no cleared indications in urology or gynecology, an ongoing inguinal hernia trial, and a regulatory pathway that hands future competitors a faster route to market all point to a back-loaded revenue story. J&J's wedge is real. So is Intuitive's moat.
The next concrete signals are the order book from the first wave of U.S. hospitals, the inguinal hernia trial readout, and any 510(k) filing from Medtronic or CMR that cites Ottava as a predicate.
