The FDA has cleared, through its de novo pathway for novel low-to-moderate-risk devices, J&J MedTech's Ottava. Ottava is a soft-tissue surgical robot whose four arms are built into the operating table rather than mounted on booms or carts. J&J calls it the world's first such system; compact rivals such as Medtronic's Hugo and CMR Surgical's Versius are likely to contest that "first" label.
The architecture is the news. Motors, sensors, and software coordinate the four arms to swing into predefined procedural positions at the push of a button, then tuck beneath the table when not in use. J&J says the layout uses 30-50% less space than boom-and-cart systems, a company-supplied figure that has not been independently verified. The surgeon console is built for upright posture and saves personalized settings per login.
Two instruments address specific pain points. A two-in-one needle driver separates "cut only" and "suture only" modes. J&J cites a company survey in which 83% of surgeons reported at least one inadvertent suture cut per month. A patented scissor keeps blade alignment along the distal third of the jaw; the same survey reports 98% encountering dull monopolar scissors.
Initial FDA-cleared indications cover 10 general-surgery procedures, including gastric bypass, gastrectomy, cholecystectomy, and hiatal hernia repair. First clinical cases are already complete, and Ottava now competes directly with Intuitive Surgical's da Vinci, the soft-tissue incumbent for roughly two decades.
