A prospective cohort of 145 premenopausal breast cancer patients suggests a far more sensitive hormone readout can flag who is likely to develop early ovarian failure after chemotherapy.
A premenopausal patient in her early 30s, newly diagnosed with breast cancer, is asked before chemotherapy starts whether she wants to freeze eggs or embryos. Today, that conversation rests on a generic warning. A new prospective study suggests a far more sensitive hormone readout, drawn at the end of chemotherapy, could give her and her oncology team a quantified risk of treatment-induced ovarian failure instead, and a clearer window to act on it.
The study, published in Human Reproduction and indexed at PubMed PMID 42664366, followed 145 premenopausal women aged 18 to 46 from diagnosis through 12 months after chemotherapy. Eighty-three received chemotherapy; 62 did not. The researchers tracked blood levels of anti-Müllerian hormone (AMH), a protein the ovaries secrete and a standard proxy for the egg reserve, at five time points: before treatment, after each chemotherapy cycle, at the end of chemotherapy, and at three, six, and twelve months after.
The clinical question was whether AMH measured at any of those points could forecast premature ovarian insufficiency (POI), the technical term for ovaries shutting down years earlier than expected, defined in the study as infrequent or absent periods combined with a follicle-stimulating hormone level above 25 mIU/ml. By 12 months, 30% of the women who received chemotherapy met that threshold. None of the women who did not receive chemotherapy did.
What makes the finding usable is timing. The strongest signal came not at baseline but at the end of chemotherapy, when treatment was still fresh. Patients who went on to develop POI had a median AMH of 10 pg/ml at that point; those whose ovarian function held had a median of 127 pg/ml. That gap gave the end-of-chemotherapy measurement an area under the curve of 0.89, a standard accuracy score where 1.0 is perfect, with 81% sensitivity and 93% specificity. Oncologists can act on those numbers rather than hedge around them.
The study used an "ultra-sensitive" AMH assay with a detection limit of 0.13 pg/ml, roughly 100 times more sensitive than the conventional clinical test. The point of that sensitivity is not a lab boast. It is the difference between measuring nothing and measuring a number, because most women on chemotherapy quickly drop to AMH levels a standard assay cannot read at all. The cohort reported a measurable AMH value in every sample drawn during treatment, where conventional assays typically return a "below detection" flag.
The clinical translation is straightforward. A pre-treatment AMH (median 5,425 pg/ml in women who retained ovarian function versus 1,103 pg/ml in those who did not, AUC 0.86) can start the fertility conversation. An end-of-treatment AMH can sharpen it, telling the patient and her team within a day or two of finishing chemotherapy whether her window is narrowing fast, holding, or still intact. That is the moment when egg or embryo freezing, ovarian suppression, or hormone-replacement decisions are still on the table, not after the failure has already happened.
The caveats belong in the same breath as the numbers. The cohort is from a single center, the sample is modest, and attrition was not detailed in the abstract. A 12-month follow-up cannot distinguish transient suppression from permanent loss; some women in the non-POI group may still decline later. The threshold for POI (FSH above 25 mIU/ml plus cycle changes) is one working definition, and a different cutoff would shift the 30% rate. And the assay itself is a research tool, not a routine send-out. Patients cannot request it at a commercial lab this week, and the study does not show that acting on its result changes outcomes such as successful pregnancy or symptom burden.
What it does show is that the timing of fertility counseling does not have to wait for menopause to announce itself. A hormone number drawn the day chemotherapy ends, combined with the trajectory over the next year, can move the conversation from a generic pre-treatment warning to a forecast. For a patient weighing egg freezing this week, that is the difference between guessing and planning.
